The Most Important Longevity Intervention May Be Subtraction

A longevity routine becomes useful when every element has a clear job—and a reason to remain.

Longevity advice almost always asks what to add things. A disciplined subtraction audit can reduce risk, recover attention and reveal which parts of a health routine are actually earning their place.

Longevity advice is built around addition. Add a supplement. Add a wearable. Add another blood panel, a biological-age test, a sleep protocol, a glucose experiment or a more elaborate morning routine.

Each addition can sound reasonable on its own. The supplement has a plausible mechanism. The device produces an interesting number. The test might detect something early. The new rule promises another small advantage.

Then the system becomes difficult to see. A person can be following 20 health rules, taking a dozen products and tracking six dashboards without being able to say which parts are helping, which are redundant and which are quietly making the important behaviours harder to sustain.

At that point, the next useful longevity intervention may not be another addition. It may be subtraction.

Why adding feels more responsible than stopping

Addition is psychologically comfortable. It converts concern into action and creates the feeling that no opportunity is being missed. Stopping feels different. It can resemble neglect—even when the item being removed never had a clear purpose.

Commercial incentives reinforce the pattern. A new supplement, scan or subscription can be sold. The decision not to buy one rarely has a marketing budget. Novelty also generates a more compelling story than maintaining blood pressure, training consistently or going to bed at a repeatable time.

The result is an asymmetry: every promising idea has a route into the routine, but few routines contain an exit.

This matters because a longevity plan is not an unlimited container. Every additional intervention consumes money, time, attention and some degree of uncertainty. Even a low-risk action can impose a cost if it displaces something more valuable.

Complexity is not neutral

Medicine already recognises this problem in a more serious form. Polypharmacy does not mean that every medicine in a regimen is inappropriate. Many people need several treatments. The concern is whether each medicine remains justified within the person’s current goals, conditions and risks.

Deprescribing therefore does not mean arbitrarily reducing the number of medicines. It is a structured process of identifying treatments whose current or potential harms may outweigh their benefits, followed by supervised dose reduction or discontinuation when appropriate. Reviews of deprescribing emphasise shared decisions, clinical priorities and monitoring—not a contest to achieve the shortest medication list.

Longevity routines deserve a similarly disciplined review. Supplements can interact with medicines, alter their absorption or metabolism, interfere with laboratory tests and create risks around surgery. The US Food and Drug Administration specifically warns that combining supplements, taking high doses and mixing supplements with medicines can produce unintended effects.

Testing adds a different form of complexity. A test does not end when the result arrives. An unexpected value may generate repeat testing, imaging, consultations, biopsies or months of uncertainty. A systematic review of whole-body MRI in asymptomatic adults found substantial rates of critical or indeterminate incidental findings, alongside limited verification data and a meaningful burden of false positives.

The point is not that medicines, supplements, wearables or preventive tests are inherently misguided. It is that the burden belongs to the complete system, while evidence is usually presented one component at a time.

The hidden cost is losing the signal

Suppose sleep improves after someone starts magnesium, changes training time, stops alcohol on weekdays, buys a cooling mattress pad and begins morning light exposure. Which change mattered? Perhaps several did. Perhaps the improvement was ordinary variation. The more variables that move together, the less the result can teach.

The same problem appears when something goes wrong. Fatigue could be attributed to a new fasting schedule, accumulated training load, a medication effect, inadequate energy intake, an interaction, poor sleep or an unrelated health issue. A complicated routine creates more possible explanations while making clean comparison harder.

This is why subtraction is not merely minimalism. It is decision hygiene. A simpler system produces clearer feedback, makes adherence easier to observe and gives high-value actions more room to work.

Research on medication regimens supports the general principle: greater regimen complexity is often associated with poorer adherence. A longevity protocol is not the same as a medical regimen, but human attention does not become unlimited simply because the goal is optimisation.

The MoL Longevity Subtraction Audit

The audit begins with a rule: do not start by stopping. Start by making the whole system visible.

1. Inventory everything that belongs to your health routine

List prescription and over-the-counter medicines, supplements, recurring tests, wearables, tracked metrics, dietary restrictions, exercise protocols, recovery practices, therapies and appointments. Include actions that feel too ordinary to count. Complexity often hides in the number of rules rather than the number of products.

2. Give every item a job description

Complete one sentence for each item: “I use this because I expect it to improve or prevent ___.” If the answer is “general health,” “anti-aging” or “just in case,” the purpose is not yet specific enough to evaluate.

Then identify the outcome that would show success. A supplement intended to correct a documented deficiency has a different job from one taken because of a cell-culture study. A wearable used to support a consistent bedtime has a clearer role than one that produces a daily score no decision depends on.

3. Separate evidence from plausibility

Place each item into one of four evidence categories: established for this purpose; supported but uncertain; biologically plausible; or primarily marketed. Mechanistic evidence can justify research, but it should not quietly acquire the status of demonstrated personal benefit.

Also ask whether the evidence applies to someone like you. An intervention studied in people with a diagnosed deficiency, disease or high baseline risk may not produce the same balance of benefit and harm in a healthy person.

4. Calculate the total burden

For each item, record the monthly cost, minutes per week, inconvenience, side effects, interaction potential and mental attention it requires. Add the follow-up burden of testing: what happens after an abnormal or ambiguous result?

Opportunity cost belongs in this calculation. A protocol that absorbs time from sleep, resistance training, food preparation, relationships or appropriate medical care is not free simply because it has no obvious side effect.

5. Look for duplication and contradiction

Several products may contain the same ingredient. Multiple interventions may target the same outcome without adding proven benefit. One protocol may undermine another: aggressive energy restriction can conflict with muscle-building goals; late intense training can conflict with sleep; an expanding supplement stack can complicate medication management or laboratory interpretation.

Duplication is often the easiest place to simplify because it reduces burden without abandoning the underlying goal.

6. Assign one decision

Keep: the purpose is clear, the benefit credible and the burden acceptable. Test: the intervention is reasonable, but needs a defined outcome and review date. Question: its purpose, evidence or added value is unclear. Discuss: it involves a prescription medicine, a diagnosed condition, meaningful interaction risk or a change that may require tapering. Remove: it is low risk to stop, has no clear role and is not medically required.

“Discuss” is deliberately separate from “remove.” Prescription medicines, treatments for diagnosed conditions and products that can cause withdrawal or rebound effects should not be changed through a self-directed longevity audit.

7. Change one low-risk variable at a time

When a non-essential, low-risk item is removed, keep the rest of the routine reasonably stable and observe the outcome that item was supposed to influence. Decide in advance how long the observation should last and what would count as meaningful deterioration.

This is not a formal N-of-1 clinical trial, and many interventions have carryover effects that make casual experimentation unreliable. The purpose is more modest: avoid changing five things at once and then inventing a story about the result.

Measure whether simplification actually helped

Success is not the smallest possible routine. It is a routine in which the important actions are easier to execute and every remaining component can defend its place.

Record a baseline before the audit: the number of daily supplements, health rules, tracked metrics and recurring tests; monthly spending; time spent administering or researching the routine; and the proportion of interventions linked to a defined outcome and review date.

After four to eight weeks, compare burden and function. Did adherence to prescribed care improve? Is training more consistent? Is sleep less disrupted by optimisation? Did any symptom or established health measure worsen? Are you spending less time interpreting data that never changes a decision?

The most revealing metric may be the percentage of the routine that has a clear purpose. A smaller stack with the same confusion is not progress. A well-justified complex regimen can be entirely appropriate.

Subtraction is not therapeutic nihilism

There are many situations in which adding something is clearly the right decision: treating hypertension, correcting a deficiency, beginning resistance training, receiving indicated screening or using a medicine with a favourable benefit-risk balance.

The subtraction principle does not compete with evidence-based prevention. It protects it. Removing noise can make proven care easier to follow, expose gaps that novelty was masking and direct resources toward actions with a meaningful chance of improving health.

Nor should every useful practice be forced to produce a laboratory change. Enjoyment, confidence, social connection and reduced friction can be legitimate benefits. They should simply be named honestly rather than translated into unsupported promises of longer life.

Give every intervention an exit

The easiest time to define a stopping rule is before an intervention begins. What outcome should change? By when? What side effect or burden would make the trade-off unacceptable? When will the decision be reviewed?

Without those questions, a trial quietly becomes a habit, the habit becomes part of an identity and the identity becomes difficult to question. The longevity stack grows because yesterday’s experiments never leave.

A serious longevity strategy should be willing to add when evidence and circumstances justify it. It should be equally willing to remove when purpose has disappeared.

The goal is not to do the fewest possible things. It is to do fewer things without a reason—and to give the important ones enough space to matter.
